SCHEDULING
We will do our best to schedule your appointment at a time convenient to you. Know that dental appointments are EXCUSED absences. Each child who is seen during school hours will receive a doctor’s school pass and signature confirming that he/she was seen in our office.
Please arrive 5-10 minutes early to your appointment so that we can assure all forms are complete and updated with the needed information. If you are over 10 minutes late, you may be asked to reschedule your appointment as a courtesy to the next scheduled patient who has arrived on time.
We know that unexpected things can happen, but we request that you provide 24 hours notice to our office if you are unable to keep a scheduled appointment for any reason.
We may charge up to $30.00 per appointment hour scheduled for failure to give 24-hour notice.

PROSTHETICS (CROWNS, BRIDGES, PARTIALS, DENTURES)
Prosthetics must be seated in a timely manner to ensure your comfort and proper fit. If you fail to have your prosthetics seated within 60 days from the date of impression, and a second impression must be made, you will be charged an additional amount of one-half of our current charge for such procedure.
